POSITION PURPOSE
The Lead Registered Nurse (RN) Case Manager is responsible for leading day-to-day case management operations and supporting the team in delivering consistent, high-quality, patient-centered care. This role acts as a go-to resource for clinical guidance, helping ensure processes are followed, documentation is accurate, and care coordination stays on track.
The Lead RN Case Manager provides ongoing support and mentorship to team members, helps identify opportunities for process improvement, and works closely with cross-functional teams to keep everything running smoothly. This position also plays a key role in maintaining compliance with clinical and regulatory requirements, supporting medical readiness efforts, and ensuring resources are being used effectively.

ESSENTIAL FUNCTIONS
  • Lead and support day-to-day case management operations to ensure timely, accurate, and patient-centered care delivery.
  • Provide clinical guidance and act as a resource for RN Case Managers, assisting with complex cases, escalation points, and care coordination challenges.
  • Monitor team workflows, caseload distribution, and productivity to ensure service levels and quality standards are consistently met.
  • Review and ensure accuracy of documentation, including care plans, case notes, and system entries, in alignment with compliance and regulatory requirements.
  • Support onboarding, training, and ongoing coaching of team members to promote consistency, performance, and adherence to SOPs.
  • Collaborate with interdisciplinary teams, providers, and stakeholders to ensure effective communication and continuity of care.
  • Utilize healthcare systems (e.g., MEDCHART, MHS GENESIS) to track medical readiness, manage cases, and maintain up-to-date patient information.
  • Identify process gaps and recommend improvements to increase efficiency, compliance, and overall team performance.
  • Assist with audits, reporting, and quality reviews to ensure adherence to contract requirements and organizational standards.
  • Escalate risks, compliance concerns, or patient care issues as appropriate and support resolution efforts.
  • Promote a team-focused environment by providing ongoing support, feedback, and guidance to staff.

KNOWLEDGE & EXPERIENCE
  • Minimum of four (4) years of professional nursing experience, including at least two (2) years in a supervisory or team lead capacity within a clinical or case management environment.
  • Demonstrated leadership experience overseeing staff, workflows, and case management operations in a healthcare setting.
  • Recent, hands-on experience (within the past five years) utilizing Army medical readiness systems, including MEDCHART and MHS GENESIS.
  • Strong understanding of military healthcare processes, medical readiness requirements, and case management practices.
  • Bachelor of Science in Nursing (BSN) or higher from an accredited institution.
  • Current Certified Case Manager (CCM) credential or equivalent certification.
  • Ability to meet federal security requirements, including U.S. citizenship and successful completion of a Tier 1 background investigation (NACI) to obtain and maintain a Common Access Card (CAC) and required system access.
  • Knowledge of healthcare compliance standards, documentation requirements, and patient care coordination best practices.
  • Excellent interpersonal, written, and customer service communication skills.
  • Personal traits of a high-level commitment, motivation, energy, team orientation, professionalism, trust, personal honesty and integrity, and a demonstration of placing others in a place of high value.
  • Must be a U.S. citizen; work sponsorship is not available.


MACHINES & EQUIPMENT
The incumbent must be able to effectively operate the current computer system, telephone system, and other office machines such as a copier.

PHYSICAL REQUIREMENTS
Must be able to lift and carry up to 50 pounds.  Must be able to talk, listen, and speak clearly on the telephone.
*All staff will work onsite at the VT Army National Guard (VTARNG) State Medical Detachment, Green Mountain Armory, Camp Johnson, Colchester, VT 05446.

WORKING CONDITIONS
The incumbent works in an office environment that is not substantially exposed to adverse environmental conditions such as heat, cold, or extreme noise.  Routine periods of being on the phone utilizing a headset, sitting and data keying are required.  Periodic times working at the office site may be required.
